The first hydroxylation occurs in the liver to yield 25-(OH) vitamin D (calcidiol). The second hydroxylation occurs in the kidneys and other tissues as well to yield biologically active 1,25-(OH)2 vitamin D.

The 25-(OH) vitamin D is the main storage form of vitamin D in the human body. It is found in high concentrations in serum or plasma, which makes 25-(OH) vitamin D the preferred analyte for the determination of vitamin D nutritional status .
